Who needs a Form OSHA 174?
Form OSHA 174, Material Safety Data Sheet is the legal form which is issued by the U.S. Department of Labor and should be used by a chemical manufacturer in order to develop a material safety data sheet for each hazardous substance that was produced according to the law.
What is Form OSHA 174 for?
Each customer of such manufacturers is obliged to receive a copy of filled Form OSHA 174, MSDS including all necessary information and descriptions on the substances which were sold. This form should be filled out by a person familiar with the terminology of the chemical, as well as understanding the basic requirements for completing this document.
This form must be accessible for all employees who are operating with this hazardous materials in their workplaces.
Is OSHA Form 174 accompanied by other forms?
There is no rule to support this form with any attachments. But the supplier must have all the necessary documents certifying the origin of the substances, their certification and documents confirming the right to possession of such substances.
How do I fill out Form OSHA 174?
This form is divided on seven sections. The following information must be provided in order to complete them all:
- Section 1: Manufacturer’s personal and business information;
- Section 2: Information on hazardous ingredients/Identity information;
- Section 3: Information on physical/chemical characteristics;
- Section 4: fire and explosion hazard data;
- Section 5: Health hazard data;
- Section 7: precautions for safe handling and use;
- Sections 7: control measures;
The form must be completed in English only and must contain the descriptions made using appropriate chemical terminology.
Where do I send OSHA Form 174?
Once completed this form must be directed to a person who received the hazardous materials from the manufacturer. A copy of this form should be kept for manufacturer’s personal records.
